Who handles your information

Dr.ShivamPT is operated by Dr. Shivam Chaudhary, PT, an independent physiotherapist in Surat, Gujarat, India. Privacy questions, correction requests or consent-withdrawal requests can be sent to shivamchaudhary.pt@gmail.com.

Information collected through this website

If you submit an enquiry, the secure website form may store your name, contact number, area or city/country, broad rehabilitation category, visit type, preferred date/time, a short note, consent status and—when Telephysiotherapy is selected—your browser time zone and acknowledgement of online-care limits. Patient reviews may store the display name you provide, rating, category, review text, consent status and an optional review video.

Why it is used

This information is used only to respond to your enquiry, assess whether the requested service is suitable, arrange care, moderate a review you chose to submit, protect the website from abuse, maintain necessary care records if treatment begins, and operate the private Admin workspace. It is not sold to advertisers.

Website visit notifications

The site's own visit counter stores a visit count, time and page path for Admin activity alerts. It is intentionally designed not to store the visitor's name, IP address or a device fingerprint in that activity record. Hosting and security providers may still process ordinary technical request data needed to deliver and protect the website.

Educational site guide

The Ask Dr.ShivamPT Guide is designed to provide general website navigation and educational rehabilitation information. Its question matching runs in your browser; questions typed into the guide are not intentionally sent to Dr.ShivamPT or stored as a patient record. If you choose Book / Enquire, information you submit through that separate form is handled as described in this privacy notice. Do not use the guide for emergencies or to send confidential medical records.

Telephysiotherapy and video calls

Telephysiotherapy is offered only after suitability and location are reviewed. Google Meet is the default provider for confirmed online sessions and is a separate service provider whose own privacy terms apply. Dr.ShivamPT does not record, transcribe or enable AI note-taking for Telephysiotherapy by default. Any such processing would require a defined purpose, a separate explanation and explicit agreement before it is enabled. Patients and caregivers are also asked not to record a session without the agreement of everyone involved.

International enquiries

People outside India may enquire, but an online appointment is not automatically available in every country. Location, applicable professional requirements, clinical suitability and the need for local in-person care are checked before a session is confirmed. Third-party hosting or communication providers may process data in other jurisdictions according to their service terms.

Minimise sensitive information at first contact

Do not upload or send Aadhaar, passports, full hospital files or other unnecessary identity documents through the public first-contact form. Keep only relevant reports available for a confirmed consultation and share them through an agreed private channel when genuinely needed.

Retention

Enquiries that do not proceed to care may be removed when they are no longer needed for follow-up, normally within 12 months of the last relevant contact. Rejected or withdrawn unpublished review material should be removed when moderation or withdrawal handling is complete. Approved public reviews remain published only while the patient's consent remains valid. If clinical care begins, clinical documentation is kept separately for the period required by applicable professional, legal and legitimate clinical-record needs.

Reviews, photos and videos

Nothing submitted as a patient review is published automatically. Publication requires moderation and the patient's consent. Consent can be withdrawn by contacting Dr.ShivamPT; public material will then be removed within a reasonable operational period, while minimal evidence of the withdrawal may be retained where necessary to document the request.

Children and dependent patients

A parent or lawful guardian should make website and Telephysiotherapy enquiries for a person under 18. If another adult depends on a lawful guardian for healthcare decisions, the appropriate guardian or caregiver should participate where required.

Your choices

You may ask what website-submission information is held about you, request correction of inaccurate details, withdraw consent for optional review/media processing, or ask for deletion where retention is no longer required. Withdrawal does not undo processing that was lawful before the request, and information may still be retained where it is genuinely required for clinical records, dispute handling or another legal obligation. Requests can be made by email using the address above. This notice is intended to give clear, purpose-specific information about the site's actual data handling and should be read alongside rights available under applicable Indian data-protection law.

Advertising, cookies and third-party links

No behavioural advertising or cross-site tracking is intentionally built into Dr.ShivamPT. Essential browser storage may be used for preferences such as language, theme and Admin notification state. Links to WhatsApp, Google Meet, Instagram, LinkedIn or other external services take you to services governed by their own privacy terms.
